PRIMARY HEALTH REGISTRATION FORM
This program is being offered free of charge for students and staff at participating schools. This will help to ensure that school reopening is done safely. This will allow the school to COVID Test students on campus should the need arise. This form will need to be filled out annually until further notice.
This process will only take you a few minutes! Be sure to include information for the person being tested when you register. If you are under the age to self consent, be sure a parent is present to consent for you.
